In 2024, Simon Hill discovered he had mild plaque in his left anterior descending artery. That personal finding drove him to ask a question that matters to millions: can supplements actually reverse atherosclerotic plaque that has already built up in arteries, not just slow its progression? After interviewing top cardiologists and lipidologists, and diving into the human trial evidence, he ranked eight supplements by the strength of the evidence. The answer is not a simple yes or no—it depends heavily on which supplement, at what dose, in which population, and measured against what endpoint.
The Strongest Case: High-Dose EPA

What you'll learn
- 1 (00:04) **Episode Introduction & Personal Context** - Simon Hill introduces the question of whether supplements can reverse existing artery plaque, sharing his own 2024 diagnosis of mild plaque in his LAD artery.
- 2 (01:40) **Omega-3 Fatty Acids (EPA): The Strongest Evidence** - Simon begins with the supplement that has the most robust evidence: high-dose, purified EPA.
- 3 (08:00) **EPA vs. EPA+DHA: Why Form Matters** - A meta-analysis shows pure EPA, not EPA+DHA combinations, reduces plaque on imaging; DHA may blunt EPA's effects.
- 4 (17:10) **EPA for Primary Prevention & Personal Use** - The VITAL trial in healthy adults found no overall benefit from a low-dose EPA+DHA supplement, but a subgroup analysis suggested benefit for those with low fish intake.
- 5 (19:58) **Aged Garlic Extract: Early Imaging Signals** - Three studies (Matsumoto, Sheikh, Wolinsky) show aged garlic extract (AGE) can reduce dangerous low-attenuation plaque and slow calcification progression.
- 6 (25:45) **Vitamin K2: The Calcification Paradox** - Vitamin K2 directs calcium to bones and away from arteries, but calcification can also stabilize soft plaque, making the net effect on events uncertain.
- 7 (33:41) **Niacin: The Biomarker Trap** - Niacin raises HDL and lowers triglycerides but failed to improve plaque or reduce cardiovascular events when added to statin therapy.

Show Notes
Can a supplement actually reverse the plaque in your arteries, or only slow it down? It is the question I am asked most, and after a 2024 scan revealed early plaque in my own left anterior descending artery, it became a personal one too.
This is part two of my three-part series on reversing atherosclerosis. In this solo episode I work through eight of the most popular supplements and rank them by the strength of the human evidence, not the strength of the marketing.
What we cover:
- Why pure EPA has by far the strongest evidence for regressing plaque, and what EVAPORATE, CHERRY, JELIS, and REDUCE-IT showed
- The REDUCE-IT versus STRENGTH debate, and why the form of omega-3 may matter as much as the dose
- Aged garlic extract and vitamin K2: what the imaging and calcification data suggest
- Niacin and the biomarker trap: raising "good" cholesterol without lowering risk
- Where CoQ10, berberine, nattokinase, and pomegranate extract really sit, plus a fact-check of a popular nattokinase claim
- My own plan after that scan, and a preview of part three on the pharmaceuticals
Only one of these eight has strong evidence behind it, and even then it is a prescription rather than a tub from the supplement aisle. Supplements can be useful additions, but never a replacement for the proven basics.
You can see exactly what I take over at my personal supplement stack.
