Alex Karnal - The Trillion-Dollar Health Revolution
April 21, 2026
AI Summary
5 min readIn 2025, the number of new people starting GLP-1 medicines each week jumped from 200,000 to 300,000 in just a few months, driven by the launch of an oral version costing $150 a month. For Alex Karnal, a veteran biotech investor who has spent two decades building and backing life sciences companies, this was the single most exciting year of his career—not because of the blockbuster revenue, but because it provided the first commercial proof that people are ready to take their health into their own hands. The gap between what medicine can do and what it actually achieves is finally starting to close.
The health stack: five layers of defense
Karnal frames proactive health around a concept he calls the "health stack"—a set of defensive layers that, if addressed early, can add a decade to expected lifespan. The five layers are lipid optimization, cardiometabolic health, neurocognitive health, inflammatory health, and blood pressure. For each, effective medicines already exist. The problem is that most people only act after disease has already manifested, rather than getting ahead of it.

What you'll learn
- 1 (03:45) **State of the Union: Why 2025 Was the Most Exciting Year** - Alex argues that GLP-1 drugs have provided the first commercial proof that society is ready for a trillion-dollar revolution in public health.
- 2 (07:05) **Introducing the Health Stack** - A framework for the five defensive layers of medicine that, if addressed proactively, could add an extra decade of life.

- 4 (09:21) **Layer 2: Cardiometabolic Health and GLP-1s** - How glycemic control and visceral fat create a brittle, dangerous system, and how GLP-1s can prevent progression from pre-diabetic to diabetic by 94%.
- 5 (10:29) **Layer 3: Neurocognitive Health and Anti-Amyloid Medicines** - Why Alzheimer’s drugs that bust plaques are promising, but only if given early enough to prevent damage.
- 6 (10:51) **Layers 4 & 5: Inflammatory Health and Blood Pressure** - How modern diet drives inflammation and how high pressure compounds the risks from the other layers.
- 7 (13:46) **The GLP-1 Inflection: What Changed in 2025-2026** - Three market discoveries that reshaped the GLP-1 opportunity: tolerability over maximal efficacy, direct-to-consumer adoption, and massive price elasticity.

Show Notes
My guest today is Alex Karnal. Alex is the co-founder and managing partner of Braidwell, a life sciences investment firm he built after spending 15 years at Deerfield Management.
The frame we use throughout the episode is the health stack. Alex talks about how most of the diseases that will claim most of our lives are already addressable with medicines that exist today. We work through the five layers of what a defensive health strategy looks like, why GLP-1 medicines represent the first commercial proof that people are ready to be proactive about their health, and why PCSK9 inhibitors may ultimately be the more important drug class even though they get far less attention.
We also get into the science and business of drug discovery itself — why most of the published literature that AI companies are training on cannot be replicated, what it would mean to have a truly agentic scientific lab running 24 hours a day, and why Alex believes we are now on a deterministic curve toward scientific superintelligence in biology.
