The episode centers on this survivor's account: a man diagnosed with terminal lung cancer who, given six months to live, chose to attempt the Subway Game as a last resort.

The Rules and the Stakes

The Subway Game is structured as a high-risk ritual with a clear reward: one wish, written on a piece of paper before the game begins, will be granted if the player succeeds. Failure means death—specifically, "the most painful and gruesome death a person will ever suffer," followed by eternal torture of the soul in the same location. The narrator, who we later learn is named Ethan, explains that he found the game while searching for dubious cancer cures on the dark web. Despite his skepticism, desperation drove him forward. "Dying now, and dying in six months would make no difference," he reasoned. "I had nothing to lose playing this game when I was already sentenced to death."
