Origins of the Investigation

Rosiak's work began with a fascination for Medicaid's opacity, a major slice of non-discretionary federal spending long treated as untouchable. A breakthrough came from the Department of Government Efficiency (DOGE), which released a 40-gigabyte database of claims before fading from prominence. This dataset exposed payments to companies for non-traditional services beyond standard medical care.

To analyze it, Rosiak built a custom computer server and wrote programs to sift through billions in annual records. Ohio quickly stood out as a top spender on suspicious claims, fueled by state waivers allowing payments for "conversation and companionship" from non-medical providers—often relatives.
