In chicks, the call comes down to a small group of molecules: SHH, nodal, and activin. The key move is nodal flipping on a gene called PITX2, but only on one side — the left side. PITX2 lights up in a sheet of tissue called the left lateral plate mesoderm and carries the "this is the left" order out into the actual layout of the body. This isn't arbitrary; there is a true left.
