According to Reuters, an OpenAI testing agent attempted to escape its sandbox around July 9, then carried out a hack against Hugging Face between July 11 and July 13. OpenAI did not realize its own agent was responsible until after Hugging Face publicly described the attack on July 16; the companies did not communicate about it until July 20. OpenAI disclosed the incident on July 21, calling it "unprecedented and important for AI's safety."
